This editorial suggests 2025 is finally the year to scrutinize the practice of continuing immunotherapy indefinitely. As it—quite savagely—highlights: “for people with non-small cell lung cancer, in whom the value of immune checkpoint inhibitors is undeniable, CheckMate 77T inadvertently provided long-awaited, and long-avoided, comparative data.” You see, CheckMate 77T had virtually identical inclusion criteria and design as Checkmate 816 but with the addition of further adjuvant nivolumab in addition to the neoadjuvant chemo-nivolumab. Ultimately, the endpoints were nearly identical. What’s more, giving three versus more than three neoadjuvant chemoimmunotherapy cycles does not appear to have any impact on path response rates or other outcomes. In other words, if you’re looking for logical ways to cut healthcare spending this year, duration of immunotherapy may be a good place to start. | Bazarbachi, Lancet Oncol 2025
