Top Line: What are the long term outcomes of SBRT for oligometastatic prostate cancer?
The Study: Metastasis directed therapy is increasingly utilized for oligometastatic prostate cancer. ARTO was a randomized, phase 2 trial of 157 patients with oligometastatic (up to 3 non-visceral sites) castration resistant prostate cancer. They received ADT and abiraterone with or without SBRT to all metastatic sites. The initial report showed significantly higher biochemical response at 6 months (92% v 68.3%), complete biochemical response (56% v 23.2%), and improved PFS with SBRT. Now, with median follow up of 53 months, the results show a significant improvement in OS with SBRT (not reached v 50 months). Radiographic PFS (44 v 17 months) and prostate cancer specific survival were also significantly better with the addition of SBRT. This is one of the first trials to show an overall survival benefit with MDT for oligometastatic prostate cancer, granted this was not the trial’s primary outcome.
TBL: Long term results of the ARTO trial show improved survival outcomes among patients with oligometastatic prostate cancer who received SBRT in addition to systemic therapy. | Francolini, Lancet Oncol 2026
