Top Line: Is adding upfront chemo to osimertinib really worth it?
The Study: FLAURA2 randomized 557 patients receiving first-line osimertinib for EGFR-mutated (exon 19 del or L858R) non-squamous cell NSCLC to +/- the addition of platinum+pemetrexed induction followed by maintenance pemetrexed. Adding to prior data demonstrating up-front chemo achieves longer progression-free survival (HR 0.62), and the analysis confirms it also achieves longer overall survival (HR 0.77). Three-year OS favored the combo (63% vs 51%), including in patients with baseline CNS metastases (57% vs 40%), and median OS was prolonged from 38 to 48 months. Upfront chemo doubled grade grade 3+ events (34%→70%), largely driven by myelosuppression, and nearly half discontinued pemetrexed due to toxicity. Importantly, most patients were ECOG 0–1, limiting real-world generalizability. Sequential therapy wasn’t formally tested, though many in the monotherapy arm ultimately received platinum doublets.
TBL: Adding platinum–pemetrexed to upfront osimertinib meaningfully prolongs overall survival, at the price of substantially higher toxicity, making it a defensible standard for fit patients prioritizing long-term disease control. | Jänne, N Engl J Med 2026
