How safe is a chemo-free approach for locally advanced lung cancer? Recently, the DART trial showed encouraging outcomes with conventionally fractionated RT and concurrent and adjuvant durvalumab in patients with LA-NSCLC. NRG LU004 was another phase 1 trial that enrolled 24 patients with stage II-III NSCLC and high PD-L1 expression (>50%). They were treated with either hypofractionated (60 Gy in 15 fractions) or conventionally fractionated (60 Gy in 30 fractions) RT and durvalumab. The combination was deemed feasible with only one dose limiting event. There were two grade 5 events, but these were deemed unrelated to treatment. | Lin, Int J Radiat Oncol Biol Phys 2026
