What is the optimal dose for treating lung oligometastases from HNSCC? In this retrospective study, 318 HNSCC lung metastases in 215 patients (30.7% p16+) were treated with SBRT. The median number of fractions was 5, and the median prescribed total dose was 48 Gy, and the median BED10 was 105 Gy. For analysis, they converted all total doses to BED20 and then to a 3 fraction equivalent dose. The overall rate of local tumor progression 6.6%. Tumor control probability modelling showed that a minimum 3 fraction equivalent dose of 38 Gy was necessary to achieve 90% control at 1 year, and a dose of 42 Gy was required for 90% control at 2 years. A 3 fraction equivalent dose of 50 Gy had >95% local control at 2 years. | Ehret, Radiother Oncol 2026
