A secondary analysis of an institutional randomized trial developed a “high-risk” group of men receiving definitive prostate radiation defined as age <70, PSA ≥10, and grade group 4-5 where having all three conferred a nearly 50/50 chance of early biochemical failure with 3 years, a risk significantly diminished with the inclusion of pelvic nodes. | Sayan, JAMA Netw Open 2023
