Here is a small series of seven patients receiving salvage stereotactic body radiation (SBRT) of 60 Gy in 8 fractions for isolated large parenchymal non-small lung cancer (NSCLC) recurrences following prior radiation. Large was simply defined as the physician feeling uncomfortable treating with SBRT over 5 or fewer fractions: median tumor size was 5.8 cm (range 3.1-8.7 cm). Planning was performed on 4D scans with 0.5 cm radial planning target volume expansion (no clinical target volume). Median disease-free survival impressively reached nearly two years (22.5 months) with a single grade 3 event being chest wall pain in the setting of recurrent tumor invading chest wall. | Ross, Pract Radiat Oncol 2024
