This case report describes a 26-year-old man who developed severe bilateral radiation necrosis 3 months following Gamma Knife ventral capsulotomy (80 Gy to 50% isodose) for refractory obsessive compulsive disorder. Imaging showed progressive vasogenic edema at 5 months despite high-dose corticosteroids—plus the patient developed steroid-induced psychosis. Enter Boswellia serrata (2400 mg twice daily). The patient achieved near-complete resolution of FLAIR edema by 9 months. Hard to prove the boswellia serrata did more than the tincture of time, but the rapid coinciding improvement (see figure 1) and favorable tolerability are worth noting. | Upadhyay, Pract Radiat Oncol 2025
