A Chinese phase 2-3 trial of 178 patients with resectable intrahepatic cholangiocarcinoma demonstrates neoadjuvant GOLP (gemcitabine–oxaliplatin + lenvatinib + toripalimab) before surgery doubled median event-free survival (18 months) compared with standard upfront surgery followed by adjuvant capecitabine (9 months). Overall survival at 2 years was improved from 61% to 79% though this did not reach significance. While longer survival analyses and broader validation will be important before routine adoption, this is certainly a promising novel strategy for a notoriously intractable disease. | Shi, N Engl J Med 2026
