At ASTRO 2025, results from the LUNAR trial showed that adding 177Lu-PNT2002 to SBRT for oligometastatic prostate cancer more than doubled PFS. Diving deeper into the full publication, more patients in the combination arm had a >50% decline in PSA (52% v 31%). Most patients in both arms progressed (86% SBRT v 64% SBRT + 177Lu), but almost all progression occurred at new sites with only 2% infield progression after SBRT and 0% with SBRT + 177Lu. Having a single oligometastasis was associated with improved PFS. This was all achieved with just 2 cycles of neoadjuvant 177Lu with no ADT. Could even better ADT-free outcomes be seen with a longer course of therapy? | Kishan, J Clin Oncol 2025
