Is pelvic radiation beneficial in patients with de novo metastatic cervical cancer? This multicenter study from Japan took a look back at 343 patients with newly diagnosed stage IVB cervical cancer. After propensity matching to try and account for selection bias, those who received definitive dose pelvic radiation in addition to chemotherapy had significantly better progression free survival and longer median OS (25 v 18 months) compared to those who had chemotherapy alone. Brachytherapy was also associated with better survival outcomes. The tradeoff, though, was a higher rate of grade 3+ bowel effects with pelvic RT. | Tsuchida, Radiother Oncol 2026
