How robust is lung SBRT local control when patients actually move during treatment? This single-institution retrospective of 78 patients treated with VMAT SBRT used pneumatic abdominal compression and a volumetric dose repainting scheme where plans were split into two symmetric halves with a 4D CBCT-guided repositioning in between. Despite course-averaged intrafraction 3D shifts up to 8.2 mm (median 3.5 mm), 2- and 3-year local control were 97.6% and 94.0%, and the only two local failures occurred in patients with among the smallest repositioning shifts in the cohort. | Walter, Adv Radiat Oncol 2026
