A French single-arm phase 2 trial evaluated ultra-hypofracionated post-operative bed only adjuvant radiation. Patients (n=90) had resected pT1-2N0-1 (no extranodal extension) squamous cell carcinoma of oropharynx of oral cavity with radiation indicated for involved (<1 mm, 30%), close (<5 mm, 65%), or uncertain (5%) margin status. The prescription was 36 Gy in 6 every-other-day fractions, and no more than 5% of the target could receive >110% of the prescribed dose (39.6 Gy) for linac-based plans. Local recurrence developed in 9% at 2 years, while roughly 15% of patients experienced a grade 3 event, namely osteoradionecrosis, soft tissue necrosis, or mucositis. | Biau, JAMA Netw Open 2025
