Top Line: It’s great to see a lung tumor shrink during treatment, but what does that mean for treatment outcomes?
The Study: This Korean retrospective study looked at 227 patients with stage III NSCLC who received definitive chemoradiation and adjuvant durvalumab. The primary GTV was delineated at baseline and again during the 4th week of treatment. Median volumes at these time points were 90cc and 64cc, respectively. Different GTV parameters were evaluated for their association with progression free survival such as absolute volume, absolute change, and relative changes. Of these, the best predictor was relative (%) change in the GTV between initial and week 4 followed by absolute change and tumor volume at 4 weeks. Median relative volume regression was 25%. Relative regression was significantly associated with post-treatment response, and it was a good predictor of local progression (AUC 0.804) and overall progression (AUC 0.711). The rate of PFS at 1 year was significantly higher among patients who had high relative regression (>30%, PFS 88%) compared to those with intermediate (>0-30%, PFS 62.6%) and low regression (0%, PFS 14.3%).
