In RTOG 1205, reirradiation improved progression free survival (but not OS) for patients with progressive GBM when added to bevacizumab. In that trial, 35 Gy in 10 fractions was used, but there are other dose fractionation regimens available for recurrent GBM. This randomized trial compared 25 Gy in 5 fractions to 35 Gy in 5 fractions in 41 patients with recurrent GBM. At 6 months, there was no difference in PFS (40% v 40%), and median PFS was 4.9 months with 25/5 compared to 5.2 months with 35/5. Local failure was 60% with 25/5 and 63% with 35/5. There was also no difference in OS. Necrosis occurred in 25% of those who received 25/5 compared to 26% with 35/5. | Chen, Int J Radiat Oncol Biol Phys 2024
