In many modern prostate cancer trials, we see both clinician rated and patient reported adverse effects. But how well do those agree? This substudy of HYPO-RT-PC directly compared clinician and patient reported adverse effects before and after radiation. Recall that the trial randomized patients with intermediate or high risk prostate cancer to 42.7 Gy in 7 fractions or 78 Gy in 39 fractions. Overall, agreement between clinicians and patients was poor across multiple time points including before and after treatment. Agreement was particularly bad at the end of treatment, and urinary symptoms showed the lowest agreement. Clinicians tended to under-report urinary and bowel symptoms when compared to patients. However, clinicians over-rated the severity of sexual symptoms compared to patients. | Ronningas, Radiother Oncol 2026
