While there may remain some debate over who qualifies for PSMA-(re)staging scans, most insurances will agree to it upon biochemical recurrence following prostatectomy, assuming the PSA is at least 0.2. Will this new imaging information alter the mountains of data we have on predicting outcomes with salvage radiation? This large international retrospective cohort of over one thousand men receiving salvage radiation after re-staging PSMA scan demonstrates all the usual suspects still predict subsequent biochemical failure: pre-salvage PSA (HR 1.80), grade group 5 (HR 2.39), pT3b+ (HR 1.91), R0 resection (HR 0.60), addition of ADT (HR 0.49), salvage radiation dose >70 Gy EQD2 (HR 0.44).
