This prospective, single arm trial evaluated ultra low dose palliative radiation for myeloma bone lesions. It included 63 patients with 70 lesions that were painful, but uncomplicated. They received either 4 Gy in 1 fraction (29%) or 4 Gy in 2 fractions with an option for an additional round at least 4 weeks later. Pain response was excellent and increased over time from 59% at 4 weeks to 63% at 8 weeks and 82% at 6 months. Nearly a quarter (24%) received a second course of RT, and pain response after a second course was 63%. | Ballas, ASTRO 2026
