The SKYSCRAPER trials have been investigating dual immune checkpoint inhibition with atezolizumab and the TIGIT inhibitor tiragolumab. We recently saw in SKYSCRAPER-03 that this combo did not improve outcomes compared to standard adjuvant durvalumab for stage III NSCLC after chemoradiation. In SKYSCRAPER-07, 760 patients with locally advanced esophageal squamous cell carcinoma who had been treated with chemoradiation were randomized to receive adjuvant atezolizumab plus tiragolumab, atezolizumab alone, or placebo. The primary endpoint of PFS was not improved with atezo + tiragolumab compared to placebo, and OS was also similar. Hierarchical testing blocked formal testing of the atezo alone arm, however both PFS and OS were significantly better compared with placebo (PFS 29.1 v 16.6 months).
