Updated ESTRO–EORTC guidelines synthesize contemporary evidence for stage I–III small cell lung cancer, reinforcing concurrent chemoradiotherapy as the backbone of curative-intent care. PET-CT staging and brain MRI remain standard across the board with involved-field (as opposed to elective nodal) irradiation and post-chemotherapy targeting emphasized. The standard twice-daily 45 Gy regimen continually holds its ground, with modern IMRT/VMAT improving toxicity profiles. Early radiation (during cycle 1–2) is favored but not dogmatic. Sequential durvalumab dramatically improves survival, while concurrent immunotherapy has yet to deliver. Finally, SBRT remains a commonsense strategy for appropriate T1–2N0 patients. | Levy, Radiother Oncol 2026
