This editorial warns of an unintended side effect of artificial intelligence creep into physician workload: “deskilling.” For instance, a Polish study of 19 endoscopists found that adenoma detection rates actually dropped below baseline when an AI polyp detection system was turned off after three months of use, warning of an erosion of the human expertise AI is purportedly designed to enhance. The authors propose “AI-off” training intervals and strict task boundaries (e.g., letting AI handle routine reads while humans focus on nuance) to preserve core skills. In other words, regular “manual mode” time may be necessary to continually exercise our clinical judgement. | Berzin & Topol, Lancet 2025
